It's a tough call between Mushishi and Ginga Eiyuu Densetsu as they are very different shows. Lately I've gearing more towards the latter as the story is simply too engrossing to beat.

For me thats an easy question, Ninja Scroll.

It was the first anime I saw (not counting Transformers as a child), I first saw it when I was 14. I still think it's the best ninja film outthere, be it live action or anime. I have an original Japanese poster advertising it from 1993 (by more luck than anything as it was in a tiny little shop in Akihabara when I went there a few years ago), a wall scroll, the OST and a tattoo of Jubai on my leg.

Though the frist two Devilman OVAs are a close second (I say the first two as I never just watch one of them).

Top 3 off the top of my head:

Katekyoushi Hitman REBORN!(though the censors annoyed me)
Toradora!
Angel Beats!

Kinda weird that they all end in exclamation points no?
EDIT: I might have more so I'll edit once I remember.

Shakugan no Shana hands down. Then I'd have to say One Piece.

Bakemonogatari and Bamboo Blade comes for a close tie followed by Toradora, FMA, and Cross Game.

Hard to say, there so many good ones. But a few I have rewatched several times would be
Ghost in the Shell
Great Teacher Onizuka
Saishuuheiki Kanojo

if i were to include special/movies then by far the best i've seen is Kenshin - Tsuioku Hen (Samurai X: trust&Betrayal)

It's a very difficult choice between Code Geass and Ghost in the Shell.

Ghost in the Shell was just a general all round masterpiece, both the films and Stand Alone Complex. I am still watching Stand Alone Complex, but I am thouroghly enjoying it.

Code Geass was, I think, not as good as a show compared to GitS, however there was more to enjoy about Code Geass. As I was watching it I loved discussing it with friends at the same stage as me, trying to come up with predictions to later points of the story by discussing some of the subtile hints left througout it. I actually managed to get the end predicted right this way it was just a fun anime to talk about.

My favorite anime is GA-REI-ZERO, its truly beautiful and ends in nothing but true sadness. The whole anime was so emotional, from happy, to sad, and B.A.
