I rarely read Derivative works but this title is quite fun. When I read this, some characters are made in line with the analysis of their real historical counterpart which impresses me. If you are a quite fond of Koihime Musou, maybe you might want to check this out.

Ah, the nostalgic, I was looking for sangokushi harem and found this, although this is a derivative or fanfic but since I like Koihime Musou so, why not? And then I give it a try, and too my surprise, this one is good. OP doesn't give too much detail I see.

I read this a long time ago so I'm a little vague with details. Several years before the start of the story, Takanashi Ayaka have a strange dream where he met with a young girl who introduces herself as "Sousou" (Cao Cao), while short, the meeting left a deep impression to both of them. A week later, they suddenly meet again and gets to know more of each other, as Ayaka wonder on what to call Sousou (she haven't received her style name "Moutoku" at that time), she tells him her real name, "Karin", and ask him to call her that. From then on although sporadic, they would meet again and enjoy their 'rendezvous'.

It doesn't take long for them to confess their love to each other and they get to know about each other; Ayaka is an Onmyouji (Taimashi) and the last living of his family, while Karin is Cao Cao with different setting than the Cao Cao from history that Ayaka familiar with. When Karin learns about the history of Cao Cao, she wishes to pursue the same path as him, to become a King. To not let Karin repeat the same mistakes of Cao Cao, Ayaka trains Karin in many fields: combat, history, strategy, etc for as the time allows them be as they know that these rendezvous will not last forever. While it is inevitable that they will part, Ayaka propose to leave a name to Karin, in hope that this name will become a beacon to guide him to meet her again. In fashion of his own name and Karin family name (as when they meet they will marry), Ayaka re-introduced himself as "Soujun Shiwa" (Cao Chun Zihe) and with true name "Raika". Then as predicted, they would suddenly unable to meet with each other.

However...

be it fate or coincidence, Ayaka would later be summoned by Karin's world (unbeknownst to him was as candidate of "Messenger of Heaven" to save that world). While Ayaka hoped that he indeed is transported to place where Karin is, he goes on a journey to learn more of the China he is in, and meet with Karin if possible. During the journey he will save several peoples that would join his cause (harem), these peoples are: Shuyu "Meirin" Koukin (Zhou Yu
Gongjin), Sonken "Renfa" Chuubou (Sun Quan Zhongmou), Kannei "Shishun" Kouha (Gan ning Xingba), Shiba'i "Senri" Chuutatsu (Sima Yi Zhongda), Kouchu "Shion" Kanshou (Huang Zhong Hansheng), Choujin "Aoi" Jungai (Zhang Ren Junyi), etc. Somewhere else, Onodera Yuuya (An otaku who has knowledge of his favorite eroge Koihime Musou) is summoned and met with Sonsaku "Sheren" Hakufu (Sun Ce Bofu). Other elsewhere, Hongou Kazuto is summoned and met with Ryuubi "Touka" Gentoku (Liu Bei Xuande).

The three will form the base of three kingdoms: Soujun & Sousou of Gi (Wei); Yuuya & Sonsaku of Go (Wu); Kazuto & Ryuubi of Shoku (Shu).

Index have only 1 MC Touma you dont think that Accelerator and Hamazura are MCs they are support characters but i agree they have a harem and it is getteng bigger but i cant see them as MCs

^ By the end of the index books (the original series, not Testament) they were pretty much as important (if not more) than Touma. Though to be fair, I don't like that multiple-MC style, I don't mind multiple POV, but having just one character to root for suits me better.

Midway two protagonist will appear, and heroines are divided between these three factions, with addition of original characters and original settings. Note that as this is written from 2012, no characters from later works will appear in this one, such as from Shin Koihime Musou Kakumei. 3. Is there snusnu in this novel? Yes, just not mentioned explicitly I rarely read Derivative works but this title is quite fun.
