The scene where you get to feel warm and fuzzy and fluffy inside when watching them? And we're talking anime, yes?
Hmm, I'd say Kotonoha hugging Makoto Amagami SS. In fact that anime is overly saturated with it one of my friends got itchy watching it. Though my personal favorite ever is the scenes between Kaoru and Aoi in Ai Yori Aoshi, and the one in DNAngel between Niwa and Riku.

Amagami SS Episode 24, Ayatsuji Tsukasa's final story arc.
Bakemonogatari Episode 12, Under the starry night sky. (Some Others Worth Mentioning...) Working!! Episode 9, Takanashi standing up for Inami. Gosick Final Episode, Victorique and Kujo's reunion. Otome Youkai Zakuro Final Episode, the 'Goodbye' scenes. To aru Majutsu no Index Episode 18, the 'Silent Promise' scene. Baka to Test to Shoukanjuu Ni! Episode 8, Minami's backstory. Baka to Test to Shoukanjuu Episode 7, The final scene with Yuji and Shouko. These are the most recent ones, I'm sure there are others that I can't recall at the moment If I were to mention my all time favorite throughout the ages... It's not even from a romance story, and it wasn't shown in the Anime adaptation. Shaman King: Osorezan Revoir - The Backstory of Yoh and Anna.

Being the fan of relationship centered stories that I am there are a good number of scenes i can think of, but there is one the impression of which hasn't faded over the years since i last watched the show.
The very closing moments of Canvas 2. One of the few anime moments that have managed to burn themselves in my memory i guess. Spoiler for Canvas 2 ending:
