Middle school friends maybe? Also now he's surrounded by three good looking girls (with a fourth and possible fifth..all A- or better on the scale) as well as Itsuki the fabulous.

I mean, he's not tapping Haruhi even though it has been suggested such a thing might end all suffering in the universe (exaggeration...maybe). He's not tapping Nagato even though she changed the Universe for him (maybe). He isn't tapping Mikuru (though he clearly want to) because of something...heat death of space-time or something like that.

Oh geez, I just noticed something else. Her uniform is different, and the only reason I can think of for that would be that it's her middle school uniform. And I really hope that's not the case because... Ew.

I would imagine he's not tapping Mikuru because she expressly forbids it, he's not tapping Yuki because she would be so compliant that it just wouldn't feel right, he's not tapping Haruhi because he's intimidated by her, and he's not tapping Itsuki because he still wants to maintain a semblance of straightness despite not tapping the other three.

Besides, she's only a few months younger there than Haruhi was at the start of the series. I don't see where your disgust is coming from.
