well, i dont think we will receive actual proof, because next chapter is the last one, but if you add the scene when she is "playing the violin" just like the first concert and then disappears, and the letter at the end shows that they are not together anymore. the manga clearly shows that she is going somewhere where he can not reach, at least not yet.
the letter could be saying that she need move to USA or Europe to do some treatment and thus cannot help him anymore or she may just have died, we will know in the next chapter,but the way the author played the entire chapter i really believe she is dead
but i really want to be wrong because i really dislike when the author kill a character like that at the end just to add drama to the manga, for me the journey is what matters and if you're dead, there is no more journey.

Kaori is dead. I really don't see the point in letting her live after the operation. Considering she would still die a couple of months after the operation, and considering the fact that she pretty much wants to end her suffering. Just please no deus ex machina bullcrap.

Kaori really was the girl next to Emi at Kousei's first concert. I guess, instead of Emi who wanted to be like Kousei and started playing the Piano, Kaori wanted to actually play with Kousei since Piano and Violin can go together. That's my assumption anyway. Still sweet though.
