If I remember correctly, the one who brought the test specimen to the lab is Seri (Shougo's father's secretary). Why did they have let Seri do that? Shougo just wanted to find out if his DNA match this suspected little sister, but they can't just go into a lab asking for a DNA test as when things turned out what they suspected, it might get leaked to the public and possibly become a scandal. So Seri brought it herself and ask a doctor to have a DNA test for anonymous people. But later we found out that she is one of the members who wants to take over the company. So God knows what she did to the specimen and it's highly possible that she fabricated the result.

Now about Miyabi, I think she misunderstood that Shougo has an imouto fetish, and so she plays along with him as his younger sister.

Interesting thought here, what if the secretary was the mother of the child. She's the one who is pushing her child on to go after Shougo. She switched DNA samples to give her child breathing room since Shougo would let his guard down and not get involved with the "sister" romantically meaning one less rival out of the way. In this case the child may not even be blood related. She told Shougo's father that maybe hoping he'd leave his wife and son for her but that failed. So now she's having her child go after Shougo, though the child is happy about this and not being manipulative like the mother, and is just using the "sister" as a means to keep him away from getting involved with the competition until her daughter is in his heart.
