I was under the impression that Fiamma just provoked Touma about what Index's reaction would be upon knowing the truth, but he never truly told her anything. Still, even if he did tell her, from what I remember Touma never had a chance to properly talk with the girl ever since the events in Vol. 18.

yap, since the civil war. They hadn't talk to each other. Being Index was in the care of Necesarius and Touma goes to war. The talk they had in Star of Bethlehem is the only talk they had science vol 18. Touma said something but before probably finishing it all. Index already knew or realize it and end the talk.

Fiamma and Index's consciousness is connected through the artifact whenever Fiamma use it.. So what Fiamma talks with Touma. Index heard it all.
