As was discussed before, the anime has elements beyond just the plain visual look to hint at the personality of the character -- things like tone of voice, and movement, subtle visual cues/gestures, music, and sound effects -- which is an opportunity the manga doesn't have. Here, they made a change that more effectively builds the suspense in the scene and helps make it a bit more believable.

The only thing people knew about the previous grail war from FSN was that Saber and Gil fought and both Kirei and Kiritsugu survived it.
Pretty much a large portion of the Sister's arc(and pretty much the overall plot) is covered in Index. Big difference there.
