No, they didn't. Volume 1 art was a little bit tooo different and it has kinda hard to recognize the character (like, I thought that Yukinon had short hair and twin tails at first), volume 2 and 3 don't have that problem, you can tell that it's Yui and you an tell that it's Yukinon (oh, and they didn't got changed, since along that new image the old cover art for 2 and 3 was kept).

Probably.
Hayama has confessed to Yukino in the past and rejected him but jealous girls "thrown out" her of the group and Hayama has not defended her.
Hayama regret now and tries to mend his relationship with her

Albeit I'm not sure how he remembered her surname in the 7th chapter of the 7th novel when they share the taxi ride and went to the movie village, which would be fine if the novel wasn't told from the Hachiman's perspective and now he forgot it. Still, he remembers the black lace from the 2nd novel.

i mean look at the two other Yukino and Yui... One shoot down anything he said.

The other Yu is simply not he type...

To only in charater in light novel series to said some thing very close to super nice in fashion much more that Yui and Yukino.
