^ I think Irish' issue is not so much lack of strength, more about the internal damage that is piling up.

And him being more of a jackass than usual is realistic, it's a normal coping mechanism for helplessness.

When someone losing weight rapidly, it means something incredibly dangerous. Anorexia was the one considered tame.

Kinda, they started like brother and sister that constantly quarrel, but something tick off after Ichika defeat her. Laura rapidly developed actual personality as time passes and leaving her abnormal, unstable fabricated emotion, which changed their relationship. The most notable example would be them seeing each other naked.

Normally, Rito has no issue baring small hint of embarrassment and protest, but later, well.... he froze just like he react with other girl naked, or totally freaked out like other girl seeing HIM naked.

EDIT: as for Ichika's harem, I'm somewhat conflicted whether to choose Houki or Charlotte as main haremmete along with Lala. Any opinions?

So the choices... boils down to Lingyin, Tatenashi.

Lun is out of question because Lala already fill the 'exotic girlfriend' quota. (not like Lingyin and Tatenashi is normal but....yeah ). She and Saki also form TLR, which mean if they're chosen, none of the canon girl has a chance.

Irish coughed and swallowed, ignoring the dizziness the urge to vomit. He could still function, albeit barely, and he wasn't going to start acting like he was some helpless cripple that needed to be helped and pitied. No, there was no time for that.

"Sarashiki......why are we doing this?"

He was referring to the fact that she had woken him up early in the morning and decided the two of them should jog all over the field as 'excercise'. Needless to say, his curent condition made it hard to do either.

"Hm? We're just getting some exercise." She kneeled down in front of him and offered her hand, "Come on, we still have more to do."

He ignored the offered help and stood up on his own, resulting on him falling on his backside, legs shaking. The condition was already worsening.

"Are you all right? You've been getting sick recently." SHe once again offered her hand.

He scowled up at her and brushed off her hand before finally managing to stand up, the dirt clinging to the gray sweatpants he wore.

"Look, just tell me why you suddenly decided to go out doing 'exercise routines.'" He said the last part with evident sarcasm, "And stop changing the subject."

She sighed and grabbed old of his arm, "You've been getting more and more malnourished the last month, I thought some exercise would do you good." As if to emphasize her point, she pulled back the sleeve, exposing the thin forearm covered underneath, "I'm getting worried."

Irish stayed silent for a few seconds before he suddenly started laughing, "So that's it? You think this is something that can be solved by stuffing my face with food and running around a field a hundred times?"

"What do you mean by that?" Her eyes narrowed in suspicion. THere was something he wasn't telling her.

"It's not important....." He pulled back his hand, nearly collapsing on his backside again. It was getting hard to breath again, he'd need to make sure that he was ready in case anything came up.

"I think it is." She insisted.

"Just because you think it is doesn't mean its right." He bit back, annoyed.

"You said this couldn't be solved, what did you mean by that?" She pressed again. She wasn't going to take no for an answer.

"....Do you really want to know?" He scowled when she nodded. Of course she'd want to know, always nosy.

"I'm dying."

"...."

Her eyes widened and she stepped back, most likely from shock. He couldn't blame her, she was probably surprised that she'd have to get another partner so soon.

".....Stop lying." Her voice was barely a whisper and lost its usual spiritedness and zeal.

"I'm not lying." He sighed and scratched the back of his head, "Anyway, you wanted to find out and you did. Lets just go back to what we were doing and get this over with. I want to go back to bed."

"Wait, that's it? You say you're going to die then suddenly you want to ignore it?"

"Ignore it?" His eyebrows furrowed in annoyance, "I haven't been ignoring it, I've known about it the minute I put that thing on. The test results clearly showed the results of prolonged usage." He smiled bitterly, "Still, the benefits outweighed the negatives and I chose to wear it. This is just the final stages."

"So that's it then? You don't care if you die? That every time you go to sleep you might never wake up?" Her tone turned angry, questioning.

He turned around slowly, looking straight at her. This was rare for him since he preferred to avoid eye contact. Still, this wasn't what attracted her attention, rather it was something else entirely:

The tears in his eyes.

"No.....and neither should anybody else." He continued to smile even through the tears.

Anyway, Tatenashi seems like the most reasonable one. Houki and CHarlotte have been done to death and Rin is basically too tsundere 9/10 times.

Adding body mass? Well, you better ask meister up there, surgery is not my specialty.

For the second one, yes, it's called corrective surgery, but not always done because of invasive nature and the risk following. It was done if medical treatment is not enough.

It's still there, but this time it was Rito who she approached. Of course, she's banging her head into the wall about the fact that Rito is halfway to be Kanzashi's friend before she add extra complexities. It turned out good though, her effort was paid decently.

Furthermore, Tatenashi also has strained friendship issue with Saki. As well as persistent misunderstanding with Motemitsu. This time though she solved it by herself s part of her own character plot

Nothing weird, remember the Parent->Spouse template I said before? Children often (though not always) use their parents as template for their spouse.

As a child, they might use brother, sister, father or mother as frame of reference. A child after all has no distinction between Platonic, Familial, Lust, or Admiration.

Incest plot notwithstanding, above situation is normal.

I have considered some idea about Kanzashi or Cecilia being main, but that means I have to kick Laura (the TLR spot is occupied by Haruna already). Incidentally, Laura was the third most common Heroine chosen, topped only by Charl and Houki.

So maybe there's a hope.

Well, there is the problem of how the Nanomachines get their energy. You could wirelessly supply them... but then you might end up pumping lethal ionizing radiation at your patient (ionizing radiation is also known as nuclear radiation). The Nanomachines could get their energy from ambient energy in the human body, but then they would work very slowly. Also, remember that the size of Nanomachines immediately make it impossible for them to do complex tasks without external guidance. Seriously, how do you cram enough processing power to a Nanomachine for it to know the difference between, let's say, red and white blood cells? Or differentiate the different types of Antibodies?

But anyway, since IS verse isn't that realistic anyway, you can just handwave that. I probably would too, if I were using nanomachines.

No. I find myself cheering for the bad guys often. My recent fanfic (still in planning stages) has a psychopathic (and not the goody two shoes fictional variety, either. An actual Psychopath) villain protagonist who quite literally feels nothing.
