I thought the romance in Spice and Wolf was understated as well...

Much of the romances I like are understated. Crest of the Stars is a great love story I think.

Hmm... if you are going to relax the action/fantasy restriction (as long as it is not childish) then there are a few more to list:

Currently airing Inari, Konkon, Koi Iroha (though I don't know how far they will get, probably not all the way to the conclusion.
Ah My Goddess (if you include the movie)
Otome Yukari Zakuro
Clanned and Clanned: After Story
Kokoro Connect
Mysterious Girlfriend X

These are not traditional "romance" genre, but the romantic relationship is at the heart of the show.
Mirai Nikki
Neon Genesis Evangelion (including End of Evangelion)

Cross Game - A Story of Love, Life, Death - and Baseball. What more could you want?

Kokoro connect
Kimikiss pure rouge it's adaptation of harem game, but they added another male to change it into two triangles).
Kobato - there is some fantasy, but in slice of live way.
Planetes - same but with sci-fi, romance feel realy good.
REC- Short and can be watched whole in one go. Adult characters and happy ending exactly how you want it.
