The "go off with x" bug is a well-known bug. Basically, the game only records the first character you started a romance with and ignores any after that.
I swear I don't have this problem. The only time Hawke really dies is when I get stupid and don't send Aveline in first, or when I use too many damaging and not enough stunning abilities, or when I don't see a Sarebaas...

Finished it few hours ago, and I'm still in that ''the hell...'' mood. Was expecting something throughout the game only to never happen. It felt to me like a half of the game is missing. Flemeth was just a guest character, and I was expecting big involvement from her.
The game was good, loved how the mage class was presented, it was badass. While watered-down at some important things which made DA:O so great, I still enjoyed it. Which I can't say the same for the ending. Seriously, I hate when I get myself into 40+ hours game only to get half-minute ending with not knowing what became of who. What's with the recycled dungeons? BioWare, that was insulting!

There are more bugs liek that. I am supposed to save a gouverners daughter int he Wounded Coast, had walked all over the map and realized they were little top of the entrance. The bandit leader popped up but he didn't do anything, no cutscene and he's unselectable. He just stands there and nothing I do engages the quest.
