I'm looking for a Movie/TV/OVA/etc and/or Manga with an (extremely) paranoid/self-obsessed protagonist.

Welcome to the NHK! has a paranoid. but non-violent main character. The manga suggests Satou's paranoia comes from using a substance with which Tony was quite familiar, but the anime does not show that. Even more paranoid is Satou's senpai from high-school days; she's a walking pharmacy when we meet her at the beginning.

Higurashi franchise should have some paranoia (didn't watch just heard)

Try Gungraves. It takes a while before it happens though.
